Hodgkin's disease HD ; represents a malignant lymphoma in which the putative malignant Hodgkin and Reed-Sternberg H-RS ; cells are rare and surrounded by abundant reactive cells. Single-cell analyses showed that H-RS cells regularly bear clonal Ig gene rearrangements. However, there is little information on the clinical evolution of HD in given patient. In this study, we used the single-cell polymerase chain reaction PCR ; to identify H-RS cells with clonal Ig gene rearrangements in biopsy specimens of patients with relapsed HD. The obtained clonal variable region heavy-chain VH ; gene rearrangements were used to construct tumorclone-specific oligonucleotides spanning the complementarity determining region CDR ; III and somatically mutated areas in the rearranged VH gene. A number of biopsies were obtained during a period of 3 years from two HD patients. H-RS cells with identical VH rearrangements were detected in two separate infiltrated lymph nodes from one patient with nodular sclerosis HD. In a second patient with mixed cellularity HD subtype, clonal VH rearrangements with identical sequences were detected in infiltrated spleen and two lymph node biopsies. The first such blank allele" was reported already in 1989 by Sutton et al. In that allele of a class II gene, HLA-DRB4, a substitution within the acceptor splice site of intron 1 gives rise to an unusually large trans.deleted. and results in no functional protein. In some DR7 positive individuals, a non-expression of DRB4 is observed. Serologically, the expression or non-expression of the DRB4 gene product, DR53, is easily identified by specific antisera. In addition, the expression of DR53 in DR7 haplotypes is highly associated with DQ2, the DR53 nonexpression is associated with DQ9. The defect gene is, however, not recognized by the conventional PCR-SSO or PCRSSP DNA typing methods because the exon sequences of the normal and defective gene are absolutely identical. The non-identification of the DRB4 defect allele may therefore result in a donor-recipient mismatch. The influence of such mismatches on graft outcome was analysed retrospectively by Mytilineos and Opelz. It could be shown that matching for HLA-DRB4 DR53 ; evidently improves kidney graft survival. DRB4 mismatches resulted in lower survival rates. The clinical relevance of such HLA defect or null" alleles remains to be clarified by further studies. However, in some allele combinations of donor and recipient the possibility of a defect gene should be excluded before the transplantation in order to avoid adverse HLA mismatches. Mayo Clin Proc 52: 11-18, 1977 At the McGill University neuro-ophthalmology unit, 52 patients with superior oblique palsy were seen during the 2-year period October 1973 to August 1975; these included patients with congenital, traumatic, vascular, and other more rare causes of trochlear paralysis. Half of the patients with congenital palsy had diplopia, requiring treatment, although surgery was rarely necessary. Patients with palsy due to trauma, the major cause in our series, included four with bilateral involvement. With the exception of recovery in a single muscle of one patient in this group with traumatic palsy, no spontaneous recoveries have been observed. Treatment modalities have been tried, including observation alone, occlusion for symptomatic relief of diplopia, use of prisms, and recession of the ipsilateral inferior oblique muscle. Four patients with palsy of presumed vascular cause recovered spontaneously within a 4-month period. All patients who were surgically treated improved.

What is clear, however, is that Edwards takes the rather narrow view that genealogical information is useful only for diseases that can be fitted with unifactorial models. We disagree. Data are the fuel of all scientific investigations, and in the case of genetic studies it is intuitively obvious that phenotypic, genetic, and genealogical data are all relevant. All methods for finding susceptibility genes with anonymous markers even those that do not assume unifactorial aetiology ; involve searching for genomic regions that patients share. Apart from power considerations, one aspect that distinguishes the various approaches is resolution. For linkage and family studies a low density of markers is necessary to find sharing, but it results in low resolution. Association techniques studies that focus on unrelated, or distantly related, affected individuals can provide very fine resolution, but a high density of markers is required to capture the sharing. Given our genealogy, at any stage of the gene mapping effort we have the flexibility to study affected relatives separated by any distance desirable and match it with the marker density required for that distance. We can perform studies that are half way between traditional family studies and association studies and avoid having to jump directly from one extreme to the other. Our extensive genealogy database allows us to fractionate the patient material, perhaps making the genetic basis for the disease in this subgroup even simpler. The proposed Icelandic Healthcare Database would anonymise and combine such data for a large proportion of the population those who give active informed consent for the genetic data to be used ; , 2 allowing one to ask fundamental questions about how interactions among genes, or between genes and the environment, cause disease. Mapping genes for common disorders is not easy, and combining extensive genealogy of an isolated population with powerful genome sharing methods represents a new approach to human genetics. But this may be irrelevant to Edwards, who seems to have contempt for most state of the art techniques.
